NAM-style form - This page demonstrates the NAM-style payment form template. Page has a contact us page with room for address information, and the check/credit card pages are selectable from the payment form itself (not the landing page). Convenience fee implemented.
Yakima-style form - This is a simple landing page linking to a simple payment form. The end user must check the box on the landing page indicating agreement before being allowed to proceed. No convenience fee.
Liberty-style form - This page has expanded landing page text, a contact us page, a checkbox to indicate agreement, and a 5% convenience fee implemented on the form.
Solace-style form - This page has a drop-down account selection menu for illustration purposes. The listed account displays a payment form. The other account displays a message stating "Please call in to make a payment." No convenience fee.
Very Customized
Very Customized Form - This page has a highly customized background, shows how well we are able to match a user's existing website. Must agree to terms before being able to move forward. Ability to select between check and credit cards at the top of the form.
